Welcome to Love Connection Transport Services LLC. These Terms and Conditions ("Terms") govern your access to and use of our non-emergency medical transportation (NEMT) services. By using our services, you agree to be bound by these Terms.

1. SERVICE OVERVIEW

Love Connection Transport Services LLC provides non-emergency medical transportation for individuals in need of reliable, safe, and timely transportation to and from medical appointments, facilities, or other approved destinations.

2. ELIGIBILITY

- Services are available to clients with valid transportation benefits or pre-approved private payments.

- Clients must be physically able to ride safely with or without mobility assistance.

- All clients must provide accurate and complete information during booking.

3. BOOKING AND SCHEDULING

- Appointments must be scheduled at least 24-48 hours in advance.

- Same-day service may be available on a case-by-case basis but is not guaranteed.

- Pickup and drop-off times may vary based on traffic, weather, and other conditions.

4. CANCELLATION AND NO-SHOW POLICY

- Cancellations must be made at least 4 hours before the scheduled pickup.

- No-shows or late cancellations may be subject to a fee.

- Repeated no-shows may result in suspension of services.

5. PASSENGER CONDUCT

Clients must:

- Be respectful to drivers and staff.

- Wear seatbelts at all times.

- Avoid smoking, eating, or drinking inside the vehicle unless for medical reasons.

- Follow all driver instructions for safety.

6. WHEELCHAIR AND SPECIAL NEEDS

- Please inform us in advance if wheelchair or mobility assistance is required.

- We reserve the right to refuse transport if the proper equipment was not requested.

7. SERVICE LIMITATIONS

- We are not an emergency medical service. For emergencies, call 911.

- Transportation is limited to pre-approved destinations.

8. LIABILITY

- Love Connection Transport Services LLC is not liable for delays due to circumstances beyond our control (e.g., traffic, weather, road closures).

- We are not responsible for lost or forgotten personal items.

9. PAYMENT AND BILLING

- Payment terms will depend on insurance coverage, Medicaid/Medicare contracts, or private pay arrangements.

- Outstanding balances must be paid in full before scheduling additional services.

10. PRIVACY POLICY

We comply with HIPAA and local privacy laws. All personal health and contact information is kept confidential and used only for the purpose of providing transportation services.

11. CHANGES TO TERMS

We may update these Terms at any time. Clients will be notified of material changes. Continued use of our services after such updates constitutes acceptance.
